RN CASE MANAGER- Dallas, TX, Part-Time (20653)

Position Title: Part-Time RN Case Manger

Hours: 25 hours/week

Position Summary:

We are seeking a dedicated and organized Registered Nurse (RN) to serve as a Part-Time Care Coordinator. This hybrid position (on-site and remote) will focus on managing the ongoing care of assigned members to ensure timely, accurate, and compliant care coordination.

Key Responsibilities:

• Complete and maintain member Care Plans on a monthly schedule.
• Participate in Interdisciplinary Team (IDT) meetings to review and update member care.
• Communicate with hospitals and other clinical providers to obtain updates on member status.
• Accurately document clinical information in the EMR in accordance with agency policies.
• Collaborate with internal staff and external providers to ensure comprehensive, person-centered care.
• Attend Team clinical meetings
  
  

Qualifications

Qualifications:

• Current RN license in Texas
• Strong organizational and time management skills
• Proficiency with EMR/charting systems
• Excellent communication skills, both written and verbal
• Ability to work independently in a hybrid setting
  
  

Work Environment:

• Hybrid position with a combination of on-site and remote work
